GENERAL PURPOSE OF JOB
The Outreach Coordinator provides assistance to QuadMed's Chronic Condition Management and Wellness programs by facilitating outreach and engagement efforts.
This is a Full-Time position with a M - F 40 hours per week schedule.
KEY RESPONSIBILITIES
- Program resource for patients and internal partners, which includes scheduling appointments and referring patients to appropriate resources
- Handles enrollment and tracking of participation as programs and services expand through new QuadMed locations
- Looks for proactive outreach opportunities to increase patient enrollment and engagement in QuadMed programs
- Coordinates mailings and other targeted outreach efforts
- Keeps inventory of educational materials at QuadMed locations across the country
- Handles procurement of supplies
- Administrative duties including faxing, scanning, data entry, etc.
- Other duties as assigned.
JOB REQUIREMENTS
Education:
- High school diploma or equivalent required
- Bachelor's degree from an accredited college/university in Human Services or Communications related field of study preferredor equivalent work experience
Experience:
- Two (2) years of experience in health care environment preferred
- Making outreach calls and promoting wellness programs a plus
Certificates, Licenses, Registrations:
- Current CPR (BLS) certification or must be obtained within 60 days of hire
Knowledge, Skills & Abilities:
- Communicate effectively, both orally and in writing
- Strong computer skills including a proficiency in Microsoft Office (MS Word, Outlook, Excel and PowerPoint).
- Proven organizational and project management ability required
- Multi-task while maintaining a high level of attention to detail required
- Work successfully with diverse populations and demonstrated commitment to promote and enhance diversity and inclusion
