Hourly Pay Range (Minimum - Midpoint - Maximum):

$81.330 - $104.942 - $128.554


The stated pay scale reflects the range that Sharp reasonably expects to pay for this position. The actual pay rate and pay grade for this position will be dependent on a variety of factors, including an applicant's years of experience, unique skills and abilities, education, alignment with similar internal candidates, marketplace factors, other requirements for the position, and employer business practices.



What You Will Do

In this role, you will be providing pre- and post-transplant related clinical services in kidney and pancreas recipients. Clinical functions shall include providing patient care as it relates to drug therapy; reviewing, researching, and developing drug protocols for recipients in the inpatient and outpatient setting based upon current medical literature and clinical experience.



Required Qualifications

  • Doctor of Pharmacy (PharmD)
  • 2 Years completion of PGY1 residency and PGY2 residency. Or
  • 3 Years completion of PGY1 residency and 2 years experience in advanced practice area. Or
  • 5 Years experience in the advanced practice area.
  • California Pharmacist License - CA State Board of Pharmacy -REQUIRED
  • ACLS Certification (Advanced Cardiac Life Support) - American Heart Association -REQUIRED


Other Qualification Requirements

  • Any BPS (Board of Pharmaceutical Specialties) Certification - Required
  • ACLS is required within 90 days of hire.


Essential Functions

  • Clinical Operations
    Supporting pharmaceutical care of solid organ transplant patient population.
    Review and make recommendations on optimal therapeutic management of solid organ transplant patients in all phases of care (pre and post transplant). Optimal review includes but is not limited to: pharmacologic and nonpharmacologic risks pertaining to transplantation; assessment of immunologic risk pertaining to induction therapy selection and/or need for desensitization; ongoing assessment of anticoagulation, drug interactions, medication allergies, and medications for chronic issues; vaccination status; and antimicrobial stewardship.
    Attend and participate in multi-disciplinary transplant rounds including selection committees.
    Provide therapeutic drug monitoring oversite for immunosuppressive agents or other agents with narrow therapeutic index.
    Facilitate transitions of care as related to medication management including discharge prescription management, ensuing access to immunosuppressants and medication education to transplant recipients and/or caregivers.
    Serves as a Pharmacy liaison with other departments, disciplines, and physicians involved in care of solid organ transplant recipients.
    Participates in developing, implementing, and monitoring clinical pharmacy services relating to transplant pharmacotherapy.
    Supports optimal use of high-risk medications for induction or maintenance therapy as well as treatment of rejection by ensuring appropriate use, patient safety and cost containment.
    Contribute to pharmacological management of living donors including pain management, bowel regimens, fluid and electrolyte management and education of aftercare.
    Respond to drug information requests and communicate with physicians and nurses in a timely manner about therapeutic issues (e.g. drug interactions).
  • Administrative Functions
    Assists in developing, revising, and implementing protocols and relating to pharmaceutical management of transplant patients (e.g. protocols, institutional guidelines, treatment pathways).
    Participates in the management of formulary as related to transplant population.
    Participate in cost saving initiatives as related to transplant population.
    Performs and coordinates quality assurance activities to identify areas for improvement in patient outcomes.
    Develops and maintains collaborative relationships with internal and external stakeholders.
    Participates in collaborative outcomes research in the area of transplant.
    Participates in the preparation for accreditation of transplant program and other regulatory agency site surveys (TJC/CDPH/CMS).
  • Clinical Consults and Monitoring
    Provides consultation and monitoring under the Clinical Practice Agreement (CPA) or under supervision of a physician.
    Certification/Recertification, Participation, Knowledge: Demonstrates competency in performing formal clinical consults, and patient monitoring practices as approved by the P and T (T and S) Committee(s).
    Has met minimum certification/recertification requirements as outlined in the Pharmacy Certification Process for Formal Pharmacotherapy Consult and Advisory Services policy and procedure for consult services offered at entity.
    Participates in Clinical Consult Services, for example: Pharmacokinetics, Nutrition Support, Ortho Coumadin, Pain Management, and other services specific to the entity.
    Demonstrates knowledge of consult services as evidenced by attainment of consult service goals and proper documentation of actions, recommendations, and patient outcomes on the pharmacy monitoring forms.
    Performs by leaving concise, objective, legible, and professional notes in the patient's clinical chart.
    Consistently demonstrates ability to complete formal clinical workload assigned during shift as evidenced by completing all consults and advisory services in a timely manner.
  • Safe Medication Practice
    Identify gaps in health literacy, markers of medication non-adherence, and pre-existing intolerances and adverse effects to pharmacotherapy that may affect post-transplantation immunosuppressant adherence.
    Demonstrates proactive participation in medication safety initiatives as demonstrated by: Safe Medication Practice Knowledge of and active participation in concurrent medication safety event (MSE) monitoring programs, as demonstrated by interventions and RL submissions.
    Identifies, evaluates and analyzes medication safety events. Performs initial analysis of MSE's and assists in the research and identification causative factors of events within transplant population.
  • Quality
    Document transplant pharmacy services provided in accordance with regulatory requirements based on Sharp Healthcare policies developed in alignment with CMS Codes of Participation and UNOS policy.
    Provides timely, constructive and relevant feedback to prescribing physician and/or pharmacy staff to facilitate optimal patient care.
    Fosters a positive work environment to support organization functions, while striving continually to find emerging opportunities for departmental, organizational, professional, and personal growth.
    Supports and facilitates the achievement of departmental and hospital goals.
  • Professional Development
    Supports staff pharmacist involvement in optimizing clinical services related to transplant population.
    Assesses the competency of pharmacists related to immunosuppression and develops appropriate performance improvement programs.
    Provides appropriate interdisciplinary educational programs relating to best practices in transplant population. Presents at and participates in staff meetings.
    Assists department in keeping up to date on changes, challenges, and opportunities in healthcare and pharmacy practice.
    Represents Sharp Health System and the department of pharmacy at local, state and national meetings.
    Reviews and evaluates key primary literature as appropriate. Keeps current with treatment practice consensus.
    Serves as preceptor for pharmacy students/residents on transplant rotation.


Knowledge, Skills, and Abilities

  • Computer skills are highly desirable, especially in the application of excel and word processing.
  • Pharmaceutical knowledge and skills.
  • Ability to learn computer and application skills as applicable to role.
  • Ability to make judgments in demanding situations.
  • Ability to manage and oversee projects involving multiple resources, from start to finish.

Sign on Bonus - This position is eligible for a $15,000 sign on bonus for qualified External Candidates.

Relocation - This position is eligible for a $5000 relocation bonus for qualified External Candidates.
